Apply for a Scholarship
If you know a child that is interested in joining a hockey team and meets the requirements outlined above, please fill out the form below to apply for a ManAdvantage scholarship. Please contact a ManAdvantage representative at (636)328-1697 if you have any questions.
The following documents must be included in the scholarship application:
- Most recent school transcripts
- The most recent tax return
- Two (2) letters of recommendation
- A brief narrative of the accident or fatality of the parent/guardian of the applicant
- For injured parents/guardians: The first report of the injury and the current medical records/rehabilitation reports
- For parents/guardians who are deceased: A copy of the death certificate
